Modernisation brought above the age of the chronic disease -- those which take decades to kill you.
The biggest implication is that you can't tell apart a healthy person from a dying one. eg: an outgoing fat guy stuffing multiple helpings of gluten-containing sandwiches is going to seem "better off" than a the focused academic following a regular regime of exercise and non-gut-inflammatory foods (a la the paleo diet).
Furthermore, many unhealthy people love to talk about their problems -- admitting to being unhealthy is seen as "normal", somehow being used as a humble (honourable?) admission of the frailty of the human will.
Let's not glorify weakness.
